TS20000118 and Adobe Form display

Good morning,

We are trying to implement the travel request approval workflow and cannot seem to get the form to display from the UWL.  The "Display Form" button in the Universal Work list brings up a truncated, blank screen instead of the travel request form.  This same form works fine from ESS My Trips and Expenses POWL as well as MSS Approval of Expenses POWL, but we need to make it work in the UWL because we require multi-level approvals.

We found and applied the notes on missing parameters TripNumber and EmployeeNumber to the workflow and have verified that these fields are available in the container at runtime bound to the task.  We did not find a task visualization delivered for this task in SWFVISU, but we have created the following there:

Can anyone spot what we are missing?
