Duplicate Work Items

Hi,

It depends on the design of your workflow.
If you do not any wait for change event that will delete the initial 
workflow item then both work items will be available.

This is my suggestion.
If the SO is not release at the time of change then this work item should 
be deleted and a new workflow started.  I say this because the SO was not 
release at all.
If a change is made after the release then the approval of the change 
should go to the position/person/user that release the SO initially.

Yes. That is what the requirement is. But it should also trigger a Work 
Item for approval when SO is changed..
