Workflow Scenario for ADOBE Forms

Hello Experts,

                I have a situation here, I need to prepare a workflow
scenario for Approve Purchase Requisition .The Purchase requisition is
to be attached to the User Decision work item as an Adobe Form
attachment. The user will open the attachment  and gives input(If any
requires).

                I am confused with how to relate Purchase Requisition,
Adobe Form, User Decision , Attachment. Please guide me if any one
already worked on this, or anyone have any ideas.
