Hi, after adding the reviewer all the agents will be determined again . Each time once approver is added/finsihed, the entire badi will be executed to check for agents. 
So in this case, container element "approveragents" might be becoming blank. So please check your container elements and how they are getting filled. 
To test the problem, keep a infinite loop in badi and add approver and do the process debugging from SM50. 
This should solve your problem and gives you complete picture.

Thank you for the reply, the system is SRM 5.5 and latest support packs have
been applied - I've logged this on OSS and will let you know SAP's
response.
 
It only appears to be happening if an ad-hoc approver is added in the
'Approval Preview' screen as the initial approver. I can see in the
workitem log that 2 approvers - both the adhoc approver and another approver
(determined by our BADI implementation) appear; the adhoc approver is assigned
as the actual agent who then receives a workitem and approves the shopping cart,
the workflow then completes and completely ignores the other approver.
 
Any further thoughts from anyone would be much appreciated, otherwise I guess
it's down to some low-level debugging of standard SAP code!

Hello Stuart.

Adding approver for n-step BADI workflow should work just fine.  It works in
our system.  Make sure you have all the latest notes applied.  There was a note
issued just yesterday for an issue I reported to OSS (BADI workflow and held
purchase orders).

Could anyone please confirm if an implementation of the BADI:
BBP_WFL_APPROV_BADI  and the Workflow Template for Approval Shopping Cart
n-Level (BADI) is created then the Shopping Cart Approval preview Add Approver
actually works OR should it be disabled if using the above scenario.

We are currently experiencing problems using the above if an Approver is
manually added before the BADI determines an Approver - in this scenario the
Workitem is only sent to the manually added Approver and once approved, no
further Workitem is created for the system determined approver.
If an Approver is added after the BADI determines an Approver, then this works
ok and 2 step levels of approval workitem are created.

If you feel like debugging, good place would be function module
BBP_WFL_DIN_APPR_FINALLIST_GET.  This function module did have an error which
was solved by OSS note 1256566 just yesterday.
