Workflow getting triggered via 'Workflow started' and not via events

Hello,

You say it is getting hung, what is the exact status of the workitem?
Has it been routed to the user's supervisor? Perhaps you could check
if the rule which is looking for her supervisor supplies a result, by
running it separately.

What release are you on?
