2 DL Notifications

Dear WUGers

Can someone respond to my issue below please? This is happening in our 46C
environment and I was wondering if anyone else came across this before.

Hi Shai

The manager receives, the LE one that he is meanto receive. I am not sure on
the second question but it is not happening always only intermittently.
There is no substitution set up at any of the levels.
In the examples where it has sent 2 notifications, it actually creates two
DL workitems, please see the attached screenshots.
