Aggregate Comments in Workflow

Gavin,
My workitem opens a transaction.
When he clicks a button Refer He should give his commnets.
How do i create an attachment with this requirement on the button click .
 
Right now i am thinkin of opening a popup where he can enter comments and store in atable.
and retrieve them 
 
Say A has forwarded workitem to B and B again to C .
C should be able to see comments from A and B.
Will the solution you propesed  work in my requirement.

Swathi,

Rather that creating everything yourself, why don't you just instruct
users to create an attachment to the work item and put their comments
there? The attachment is visible in subsequent work items and in the
workflow log. It's simple and easy and (in my experience) under-used.
