My previous site were horrified when they found out how wide open to abuse forwarding is. Actually, you know who I'm talking about since you worked on their SRM implementation :-) 
 
We disabled forwarding alltogether for some steps. Fortunately we managed to sidestep custom developments (with a few near misses) by a number of radical design changes through the project - one of the reasons it took somewhat longer than planned. 
 
In the end it was ONLY master data that could determine where the WI goes (or SWIA). It was invoice verification, and if the approver is wrong, they have to reject it and AP have to get the org chart changed and resend for approval. The downside: some pain for AP, the upside: strict control and very good org maintenance (I hope.... they've been live for two months). 
 
It would have been good to restrict forwarding by something more than possible agents (all approvers ends up with the same issues as Mark described). 
 
Incidentally the same issues arise time and again with substitution. Yes I know we can use classifications, but they don't work for personal subs and we can't always use the positions to assign agents (e.g. document creator = SAP Userid).
 
My contribution to the wishlist is to have a third 'forwarding and substitution agents', in addition to actual and excluded agents in the WF builder. Preferably able to use a rule. 
The reason it has to be in the WF rather than part of the user/inbox is that different tasks need different forwarding; salary reviews should not go to the secretary, and a few pencils don't need to be approved by a board member.
