Shopping cart not getting approved

Hi,

I am having a strange problem with the shopping cart
item based approval workflow. When approver approves
the shopping cart, it disappears from the inbox, but
it comes back immediately. And also in the workflow
log it shows the workitem as completed, but adds the
same approver again. Its kind of going into the
infinity loop. 
Please advice if anyone has come across this problem.
