Work Flow - table Updation - Roll back Problem

Hello All,
 We are facing a problem where a work flow is ending in an error saying the 
roll back has occured in a step in ehich a data base table is being updated 
( attached screen shot) .
 The work flow didnt stop at the table updation step, but executed the 
consequent steps and then entering into erron state.
 In addition the work flow didnt stop when the same table is updated in the 
first few steps of the same work flow.
 we are 4.6c.
 Could you please help me to solve this.
