Hi Raju,
Patch levels look ok.  Sounds like one of the old problems that has already been fixed.
To be on the safe side, delete all offline content of the web browser being used and try it
again.
If its still happening, report to OSS.

Hello All,
 
Looks like I am having too many opportunities in EBP environment. When an
approver approves a shopping cart approval workitem using standard workflow
from the details window, the workitem does not leave the inbox unless the
approver approves again by clicking  the check icon next to workitem. This
is a standard workflow. Has anybody faced this behavior? It looks to me that
first approval (from detail window) sets the WI in 'Reserve' status and upon
second approval it is executing it.
