Custom Event triggering in CRM

Hello CRM workflow gurus,
 
When a service request in CRM is created it is triggering two same events
(BUS2000116 Created) at the same time. So the workflow I have developed is
starting twice.
 
I would appreciate letting me know, if anyone experienced this problem and
how you have solved it. If not, please let me know where to put a
SWE_EVENT_CREATE function module to trigger a custom event in CRM documents
or anyother way to trigger a custom event.
