Duplicate workflow item

Please simulate event  to check if  more than workflow with the same  =
event
is triggered.
 
By the way, what object are you using ?=20

 we have a workflow.in our client 450,it works
correctly,But in oue client 490,it can not,each step
the use can get two workflow item,one is reserved and
another can executable.
 
what happened?
