What version are you on ?
We are experiencing the same in a 3.1h environment.
Check in the workflow log for a return code of 134 ??? on the task.
Do not have a resolution, just beginning to investigate.

Need your expertise here.
 
My client has implemented WS10000051 within their SAP System ( FI Parked
Documents).
They have 2 levels of approvals. Managers have made released/approvals at
both levels.
 
My problem here is inconsistency whereby the standard SAP Workflow does not
kick off the next subworkflow of account assignment(WS10000055) which
basically sets the Release Flag to 'X' to  enable 'Posting' to  complete
the entire workflow.  I do not receive any errors but the status of the
workflows just reside in status 'InProcess' and therefore does not 'Post'
the Parked Documents which have been approved at both levels.
I have to manually go into the object FIPP method to set the release flag
and manually post  the FI Parked document that is hanging to overcome this
problem but this is not proper.
 
The majority of their Parked Invoice documents(90%) gets completed by the
workflow except for 10% of it which just hangs in the system.
 
Any ideas what's the cause of this 10% workitems? I've logged in an OSS
call before and SAP could not really find a problem with the workflow at
all the last time as the standard has been used.
 
I would appreciate any input or help asap!
