If you want to do this through SAPgui:
What I would do is copy the exisiting object including screen/form. Adapt
this for your needs and move the relevant fields from the screen back into
your workflow container (the fields you describe into your email and any
additional fields recuired to generate a new cost centre).
 
Then use this as the input values for a background Costcentre.create method;
after approval of the new cost centre within the workflow. If an object
which does this does not exists in the BOR you need to create the object
which creates the cost centre.
 
If you really want a manual cost centre creation the only thing you need to
do is adapt the form to store the fields you require, and adapt the workflow
for your business process.

Do you want to use SAPForms (VB, outlook) or use the sapgui to base the form
on ?
Sounds like you need a new object/method in the BOR to achieve what you
want.

     How can I create my own structured documents (forms)
     linked to an approval process?
 
     I analyzed the given SAP example "Notification of Absence", it is
     using a business object called "formabsenc". I would like to use the
     same process, but I need a different form.
 
     Form Example:
 
     Company Code:
     Proposed new Cost Center:
     Cost Center description:
     Cost Center Owner:
     Reason why:
 
 
     That form should carry some text fields to be fill out by the
     initiator. Once the form (or a document template?) has been filled
     out, it will go through a basic 1-2 level approval process. Once
     approved a SAP user will create manually a new cost center.
 
 
     Any suggestions?
