There are no rules that hold for everybody all the time.

"So if you are any good you will be in the outer lane, close to the tables"


Tete was obviously good, and could dance with whomever he wished. 
Nonetheless, he often chose to dance in the middle and not on the outer 
lane because of his preferred style of dancing (particularly the vals). 
Others did the same (I am not talking about foreigners). Why people 
choose to dance along the outer lane which is sometimes extremely 
crowded is hard for me to understand. Of course, when there is a little 
space, it is different. Ultimately, while there is general 
etiquette/codigos, they are evolving, have always evolved, and always 
will. These "rules" are also somewhat a function of location, day of the 
week, etc.

You would be dumb not to spot an invitation from at least 2/3 women each time you go round the floor. So, yes, the woman does the first move; the man eventually responds; if he doesn't, he either doesn't want to know, or he is too dim&  hasn't noticed, In which case she doesn't want to know him either&  good riddance with no time wasting being tortured on the floor.
