Comments Box on the UWL

Hi Folks,

I'm looking to add a comments box on the UWL which will allow my user to 
enter some comments before they either Approve or Reject and pass those 
comments back to the WF.

Is there any good step-by-step guide on how to achieve this, particularly 
around what's required on the portal side. I've seen a few things:
