Look at table SWWLOGHIST.   It contains entries like the one below for every work item that is forwarded.    You will still need to look-up the agent for the work item, but this should be a good start.

Hi,
We are using deadline monitoring and I need to know if there is any function module that I can use to find last agent work item was forwarded to? I have to code something to send an email to the manager of that person who was forwrded that work item and that person did not complete the work item.
