Training workflow issue - WS12000006 - User cancelled maintenance problem

Hello Experts,
We are facing an issue, while executing the approval request for booking a training request from Portal. 
 
"User cancelled maintenance; no change executed"

We are using the workflow template - WS12000003 (Workflow) & WS12000006 (Subworkflow) & The standard task - TS12000006 - Book Participation in Dialog in WS12000006.
 
We are in ECC 6.0 EHP4 and EP.
 
Please write any suggestion.
