Unable to process WI from Portal

[Srivastava, Pawan]

Hi All,
 
I have tried to use UserDecisionHandler for one of the task so that User
decision with mandatory comments can be implemented from both portal and
SBWP. 
I am now getting an issue "User decision 000000294592 has no alternative
to index 0000 " while executing the WI from portal.
WI can not be processed and is in reserve status.
 
How to fix this issue?
Any help/thoughts from anyone would be greatly appreciated.
