Work Item Repeating

[BUNMI OLUFEKO - SAP COMPETENCE CENTRE]

A process(invoice document)  that we have created runs through two approval process (decision steps) before it goes for release .

But the issue here is that the second level approval gets a decision step but when she clicks on approval it creates the same work item in her inbox.

What could be the problem?
