SAP Shopping cart issue - help requested

Hi All,
   
   I am using a copied version of the one step approval workflow template (01000129).
   
  I have created a shopping cart with 2 line items, as per my org. structure, the workitem reaches the "Manager" for the shopping cart approval.
   
  Although 
  1. The workitem is cleared from the manager's inbox, when the approver/Manager
   approves one line item and rejects the second line item.
  2. The workitem is cleared from the manager's inbox, when the workitem is approved
   at the header level, without checking the line item details.
   
  Problem
-------------
  1. The workitem is not cleared from the manager's inbox, when the "Manager"
   approves both the line item..at the item level.
   
   
  Please help me to resolve this issue, I am using the standard task & template.
