About adding notes to a user decision

I would like to attach notes to a user decision
for a reject decision, the reasons involved and other data has to be entered (mandatory) for a reject decision
What element or task is available?

You must know your password to change your options (including changing
the password, itself) or to unsubscribe.  It is:

  123456

Normally, Mailman will remind you of your mit.edu mailing list
passwords once every month, although you can disable this if you
prefer.  This reminder will also include instructions on how to
unsubscribe or change your account options.  There is also a button on
