I don't know if there is more for you to do with the Sapgui installation, but as for Outlook 2003:
you should find the right place under extras-options-E-Mail-Setup-Mail Accounts. There is an assistant to guide you through adding a new account. For "Server type" you choose "additional server types" (?, the last option in the list), and on the next screen you should find the MAPI. Then follows the configuration described in the excerpt you added.
 
However, I found that with MAPI, Outlook (at least the 2003 version) only opens if it can get a connection to the configurated SAP system. Otherwise, you don't get access to your Outlook at all. So I decided against it.

	Hi all,
	
	I'm trying to receive work items in my MS Outlook Inbox. The basic necessity
	is to install MAPI service provider from my SAP Setup.
	
	1. I'm unable to find any placeholder during the Setup to install these MAPI
	services onto my computer. I'm installing SAP GUI 640 on my PC.
	
	2. I can only see one 'Outlook Integration' checkbox during my set up. Is
	that enough to intall MAPI services on my desktop.
	
	3. I'm unable to find the exact place in outlook where I should add the
	''SERIVCE" required in my profile to actually connect to SAP.
	
	I've attached below an excerpt that I took out from SAP WUG messages only.
	This service option - I"m unable to find in my outlook. Please instruct me.
	
	I'm using MS Outlook 2003 Serivce Pack 2. Is there anything extra I need to
	do to get this thing going.

	The setup of linking SAP inbox to the Microsoft Outlook is the following:
	
	You have to install MAPI by run SAP gui setup and on the component screen
	you
	have to choose the Desktop interfaces and click on the Change options button
	and choose SAP MAPI Service Provider. After the installation you have to add
	this to the Services option in the Outlook Tools menu. Here you will have to
	give all the logon informations for the required SAP system /system, client,
	user and password/. Finally on the configuration screen choose the Message
	store sheet and tick the Workflow enable function.
	
	The result should be that a new Inbox /name: 'system nr.'+ 'client'
	SAPoffice -
	'user'/ would appear. The Inbox folder will contain the content of your SAP
	inbox /you can set up online and offline link/. The message subject field
	will
	contain the task description, the message body will contain the long text of
	the task description and the execution buttons, which by doubble clicking
	logs
	on the SAP system and goes directly to the required transaction. After
	executing the message will dissapear /online link/, just as in the SAP
	inbox.
