Workflow from Past triggered

Hello,
 I am kind of new to workflow,
We have a problem right now,
It appears that there are travel requests from 2003 that need to be
approved. When you look for the request it can not be found. It appears to
me, that something happen in the workflow that triggered old approvals.

Have you ever seen a workflow task triggeres from the past.?

I checked the workflow log, everything is dated back to 2003,

So am not sure what triggered it.

Any input will be appreciated.
