Workflow Error - SWP_CALLBACK_WI_DONE

I usually see this error, "006 rollback work," in conjunction with
another error, such as "826 object not found."  Check the log to see if
there is another error occurring from which the workflow is trying to
recover.

Hi,

I hit a workflow problem that I don't understand and know how to solve.

The first problem is that when I view the errored workflow via the
graphical workflow log, the system is displaing a view that doesn't
reflect the current template definition. The odd thing is that the
Workflow Log (View With Technical Details) is displaying the sequence of
steps. Is this normal?  

The second problem is that the workflow keeps erroring out with the
following error messages that I don't understand. The error messages are
'ROLLBACK WORK executed (SWP_CALLBACK_WI_DONE item 8) and ROLLBACK WORK
executed (SWP_CALLBACK_WI_DONE item 1')

Based on the information from the logs, the process is stopping at the
start of the  approval step.  

Can someone please provide some input regarding these problems
especially the SWP_CALLBACK_WI_DONE error message.
