Locked workitems

Hi, we have a requisition workflow running in 4.7 which now appears to
be occasionally preventing users from approving requisitions.  When
reviewing the technical log a message 'Work item X locked by user Y'
where X and Y are the requisition and approver respectively.   We are
using a header level release strategy with a custom approval screen.
Our workflow has been in production for a couple of years now and this
type of error seems to be new.  Does anyone know off hand what could be
causing this or which tool I could use to determine the locking
conflict?
