Issue in Rule-Evaluation

I didnt find any excluded agents in both the steps.
when I restart the failed workflow for 2 times, 2nd
time it is completed.
the steps assigns an attachement to the work item. I
didnt find the attachment in failure step. but if I
restart for 2 times, it is successfull..
