Problems with Change document transports

Hi All,
 
I have workflows in the EH&S area which triggered off of a few change
document objects.  Unfortunately, subsequent to transport to our QA
system it appears one of the change document objects is not working.
The change document is not being created for the object in question (in
this case CBIH_ACH) whereas it is working a similar change document
object.  From what I can tell in the 'Generation Info' tab (under t-code
'SCDO' -- select object CBIH_ACH --> click Generation Info tab button),
the main difference in our QA and DEV systems is the Dictionary
structure VCCIHT_ACH that should be associated with change document
object is missing.
 
We are still trying to track down the transport issue and contacting SAP
at the same time as we believe out of sequence manual client config in
EH&S is causing the problem.  I was wondering if anyone else on the
forum had run into an issue where a dictionary structure for a Change
Document failed to be associated subsequent to a client transport and as
a result workflow events were not generated.  Solutions to similar
experiences would also be helpful.  We are on Enterprise 4.7 extension
set 2.
