Strange behavior with DECISION step 4.6c

This more than likely what happened. It was late on a Friday night and blurry eyed. 
I was copy and pasting form an existing Task to keep standard format of text style. 
Provably did a "Cut" verses copy text. 
I wrote it off as my bad....

    Make sure there's no bindings filled between the Task & the Method. The
Std. Method 'Decision.Process' has a problem if bindings are filled in
there, & SAP won't look into it because it's considered Custom when we copy
it & use the copy. So if you have any bindings between there make sure you
use the Red Line Delete button, not just delete them by erasing the text, to
work properly. Remember, if you leave all bindings blank anytime then all
bindings by default are automatically Bound. Let me know if it helps.

I have a DECISION step with three outcomes.
Approve, Reject, Complete and the standard cancel and keep in inbox.
 
I get to this DECISION step and all I see the cancel and keep in inbox
option.
In the template, I have them all there and green checked. i.e. activated.
When I Iook at the running WF log, I see that I have these steps in the
graphical view.
 
They just do not show me these options to select from the decision step.
 
Any clues?
