WorkItem not Created

Hi All,
 
I have problem with couple of Worklows used for Purchase Requisition. One
is used for sending Approval user decision workitems as per relase strategy
and other is used for just sending notification mail to approver to logon
to SAP and approve the requisition to inbox which is fwded to internet
address of user.
Both the workflows contain contain the first task as background task, in
case of notification workflow it contain only one task of sending mail that
too in background. There is no agent assignment the name of approver to
sent mail is taken from database.
The one used for sending user decision task has first task as background
task used to get next release code.
 
Both the workflows trigger from ME51N with events
RELEASESTEPCREATED------------User Decision Wf
a custom event REQCREATED-----------mail sending workflow.
 
When seen in event trace both events are recieved properly but WorkItem is
not created(as checked in SWI2_FREQ and SWI1)
 
Or when tried to invoke the WorkFlow in Test environment then workitems are
created for background task but they remain in ready state forever.
 
 
Any ideas or help is welcome
