Workitem error - workflow administrator is not notified

Hi all,
 
We have a workitem that has an error status. It went back into the
initiator's inbox instead of the workflow administrator's inbox. I've
checked the workflow template header tab, "Responsibs." and the "System
administrator for workflow expression" is empty. Are there other ways
to override where workitems in error are routed?
