PR workitem appearing many times in inbox

Why don't you just remove the trigger event from your task and leave the
task on the end of your workflow. When the process goes there, the message
will be sent to the originator.
 
If the workflow passess on this step several times, you can put an If
statement to check if the PR is completed released before to send the
message.
