Workflows not being triggered

- do you use start conditions / check FMs? If so, check these...
- in the case of your own code starting the WF -- do you COMMIT WORK after
the call?

I've created 2 new workflows, one using status management to create the
event, the other using custom code.
Both workflows work fine under test (SWUS), they fail to be triggered in
normal operations.
Event simulation starts both workflows; create event starts both workflows
ok. Existing workflows are working normally.
Both workflows have status 'released'. I've checked tables SWFDEVENA and
SWFDEVTYP, all entries are correct and show no errors.
I'm sure (and I hope) it's something relatively straightforward.
 
Any ideas?
