PR Release WF Changed

Hi everyone, I have done some changes to my PR Release Workflow, and now I
cannot get my first task, a decision task, to work.  The decision is to
either approve or reject, very simple, however when I test it all I see in
the workitem is the 'Cancel and leave in Inbox' Selection and nothing else.
I have checked and checked, I cannot see what is wrong.  The work item does
not have my decision options although it is in the Workflow Template.
