Workflow errors not reported in SAP mail?

I believe we are having the same problem in our 4.6c system also.  I found
OSS note 375994 which is supposed to correct this problem in a 4.6c and
4.6d system.  Is this one of the notes you tried?

Hi,
I've noticed thia in a 4.6c system, found and applied some notes to that
effect, but still many errors are not mailed.
As I usually work with SWI2_DIAG/ADM1, thia hasn't been a priority so I
didn't really get too deeply into the cause.
