Workflow Intermittently Not Creating Workitems

Hi WF Group,
 
In 4.0b I am experiencing an intermittent workflow failure to create
workitems.  The workflow is the SAP standard workflow for purchase
requisition approval.  I have noticed that over a period of days a number of
non-consecutive requisitions do not generate workitems.  However, when using
SWUS the workitems generate properly.  Can anyone provide me with some ideas
of areas to look for the root cause of this issue?
