BSVW - Event Creation

I have created an event through BSVW - Status Management for an user status on an Operation (AFVC_PM) and it is working fine in our DEV environment but not in TEST environment.  
 
I have event trace turned on for all objects. 
Event create AFVC_PM (a system status) works fine. 
I have given the user account that completes the operation (user status) sap_all and sap_new to make sure it is not a security issue.
I have verified the configuration for the event create did transport to the TEST environment correctly.
I have verified the configuration for User Status is the same in both the DEV and TEST environments.
 
Any suggestions on what I should be looking for?
