Work items not getting sent to the actual agents

We have a WF for releasing Purchase Requisitions.  I have difficulty with
two users in particular who intermittently do not receive their work items.
When I check everything seems in order
Release Code
Correct user assigned to position
The recipient is listed in the Actual Agent but still does not receive the
work item.  The task is a general task.  Does any one have any ideas?
 
Restarting the workflow is the only fix, then everything is fine.
