Decisions of decision step not shown at runtime

Hello,
 
Can someone help me out: I have a decision step to which I added an extra
decision and next at runtime none of the decisions exept the standard
'Cancel and keep in inbox' is shown... I tried to delete the step and again
added it to my workflow, but no effect. The user decision task is a copy of
the standard. I created a new copy, added it to a new user decision step,
also no success. We are on 4.6C.
