Duplicate workflow item

Check how many workflows are triggered ?

 Hi everyone:
 
 we have a workflow.in our client 450,it works
correctly,But in oue client 490,it can not,each step
the use can get two workflow item,one is reserved and
another can executable.
 
what happened?
