Expense approval not carried out?

Hi all,
 I have a puzzle for you.
 
Production workflow is for Travel expense approval.   The approval work
item is sent to manager.
Manager says he has approved it, but approval status is not updated.  Only
complication is the approval is carried out late.
 
Within the approval step the actions carried out are as follows:
 
AGENT EXECUTED ACTION
Initiator         Dialog work item created
WF system   Deadline reached
WF system   SWW_WI_DEADLINE_CALLBACK
Approver    Work item reserved
Approver    Work item process complete
 
Does anyone have any idea how the approver can miss out the Execution
started action and therefore Trip approved result?
Workflow stops and does not create any more steps i.e. does not end.
Workarond in place is to retrigger workflow (this works ok 2nd time).
