Image-Assigned is not terminating TS30001128

Thanks Tim.  This got me started   I ve spent time debugging and I find that
when a duplicate record exists, an entry is created in a log table
(TOA_L03).  On the offending work items there are no entries in the log
table and the entry in TOA01 exists.  So, it seems the only thing that is
not working is after successfully completing the check fm,
SWW_WI_COMP_EVENT_RECEIVE is not successfully updating the status of the
task.
 
Unfortunately, I can t debug the exact process because it happens randomly
(1 out of 200 entries.)
 
Any other ideas of what I could look at to see why a workitem is not ending?
