Rescheduling Deadline Monitoring after system restart

We had several workflow instances go into error because the Deadline Monitoring
job (RSWWDHEX - transaction SWWB) was not rescheduled following a shutdown and
startup of the system. This happened on our Integrated Test system which was
copied over from the Production system.
We are on R/3 Release 4.0B.
 
The foreground task which was subject to Latest End deadline monitoring was
created in the users' inbox, however, the SubWorkflow which created the task
went into error. The users  executed the asynchronous task but the task did not
terminate thru event linkage. I suspect this is because the workflow was in
Error status. My attempt to restart the Subworkflow after scheduling the
RSWWDHEX made matters worse as it raised a second foreground task. I was forced
to do a lot of manual corrections to fix the problem.
 
My chief concern is that the despite the error the foreground task is created
and the user is able to execute the foreground task. If the foreground task
failed to be created, I could minimize damage by restarting the workflow later.
 
I also did not get any alerts in SAP Office that the deadline monitoring was no
longer scheduled. I only got the notices for the Workflow instances which went
into error. Ia am concerned that hundreds of workflows may go into error if it
happened at a time when I was not able to login and monitor my SAP account.
 
This is my first attempt at using Deadline Monitoring, so I don't know all the
tricks and features available to try and make this bullet-proof. Can you share
your experiences as to what you do to minimize this problem? Specifically, what
do you do  in the folowing cases:
 
1.   How do you ensure that Deadline Monitoring remains scheduled after a
shutdown? Do you ask Basis to reschedule the deadline Monitoring?
2.   How do you get alerted that Deadline Monitoring has become unscheduled?
3.   Under what conditions does Deadline Monitoring get unscheduled and require
manual rescheduling.
3.   Is there an option to prevent the foreground task subject to Deadline
Monitoring from being created, if deadline monitoring is unscheduled?
