Try refreshing the organizational plan. You can either:
 
- execute transaction SWUS - press 'Refresh organizational environment'
- go into the Business Workplace (SBWP) -> Settings -> Workflow Settings ->
Refresh organizational environment
 
I believe the org. data is buffered - this updates the buffer contents.

I have implemented a PO Approval workflow with SAP 4.6C. One of the managers
does not receive workitems in his Workflow inbox. If I execute SWI5, I can
see the workitems for him. If he executes ME28, he can pull up the POs
waiting for his approval. In the release strategies, the release codes are
attached to positions. I have checked and rechecked - he does hold that
position.
