Why dont you try and look at the log for those two work items. You should be
able to see whats going on then.
 
I am not familiar with 3.1H so I am unsure whether this is possible (I
suspect it would be).

Thanks Alon.  I am in 3.1H and it does not recognize transaction codes with
more than f letters.  I did manage to get the WF to trigger, but now I have
two workitems in my inbox.  Where are they both coming from?

check transaction SWETYPV and ensure that the receiver status is NO ERRORS
(and of course check that the linkeage is still active).

Good afternoon wf'ers.  I have created a workflow that tests ok.  Everything
works, including triggering it with the event in the test environment.
However, when I create the event by actually using the transaction, the
event log shows my WF as the receiver and that it has started successfully.
But I get no message in the inbox and I get no trace of it starting.  What
could possibly be happening????  Help, I am about to give up!!!
