Workflow in 4.6c - Error WL 411

Hi folks,
 
We are on 4.6c, and I received the following error message.
 
Error WL 411
 
"The following error occurred in the workflow above:
Workflow ' 5927306 ' work item 5927308 : No successor defined for event '
0000 '
 
Please repair the suspended workflow...
 
 
I have never seen this one before.  The step in question has three
outcomes, which are followed by a container operation.  I even manually
changed the container to have the value I want, and restarted the workflow,
but got the same error again.
 
Any help would be appreciated,
