Text display for Work Item

Hi,
Thought someone might have encountered the following:
 
Occassionaly different text messages for a workitem are displayed
for the same exact task within the same exact workflow definition
(same step).
 
The difference is that sometimes the workitem text will be
displayed - while other times the tasks name will be displayed
instead.
 
Does anyone know why this should happen and a fix?
We are on 4.6B.
