Workitems not routing to approvers inbox

Hello,
 
We're on 4.5b using workflow for purchase req. approvals, with 27 primary
approvers.
 
We've changed a release code/strategy to a different user and assigned the
new agent ID in OMGQ WF.
Purchase requisitions were created and shows this new person in the release
strategy (ME53).  The new approver can approve manually.  Workitems are
being generated, but they're not routing to her inbox.  Mail is routing, not
workitems.
-Org. assignments (position)are correct
-Refreshed org. environment in WF inbox of the approver.
-Authorizations are assigned
 
Everyone else works fine.
 
Any insight as to why workitems are not flowing to the inbox or where else I
can look will be greatly appreciated.

P.S.  I've been doing WF for 3 months and have run out of ideas on this one,
