Event Linkage Keeps Deactivating

I have a live workflow in which one task event linkage keeps deactivating. Recently it looks as though it deactivates each time it is called, however, so far I haven't been able to prove this in every case. Does anybody have any ideas why this would happen. Whenever I check the task binding, the consistency check is OK. The entry for the task in table SWETYPECOU always exists.
 
Also, when I activate the task I get an SAP error message "Type linkage between <OBJECT TYPE> <EVENT> and <TASK>: Insertion not possible". However when I return to the Event Simulation screen the task event linkage is then activated.
 
Any Ideas?
